Yes, you should document all employee changes; hire, job title, salary/hourly rate, leave of absence, termination/resignation! Include the effective date, who made the decisions, and a spot for the employee to sign as needed. It ensures that you and the employee are on the same page and that documentation is in place should questions come up.
